Home Health Aide

Adaptive Nursing and Healthcare Services, Inc. is a fully licensed Home Health Company specializing in long term in-home care to patients with significant medical needs.

While many companies exist that focus on skilled nursing and therapy services, we focus our attention on the aged and disabled population who need more than intermittent skilled services, and specifically benefit from daily help with Activities of Daily Living including:

Companion Care Services

Adaptive Companion Care Services, Inc. is a fully licensed Personal Services Company specializing in providing long term non-medical care to help patients maintain independence in their home for as long as possible. These non-medical services can be used in conjunction with other services offered by Adaptive, and are based on the patient’s needs.

Services may include:

  • Attendant Care Services
  • Homemaking Services
  • Transportation
  • Errand Running
  • Companionship

Adult and Pediatric Nursing Care

Private Duty Nursing (PDN) is 1 on 1 care between a licensed nurse and patient. It allows medically fragile patients to receive quality, skilled nursing care in the comfort of their home: a setting preferred by most patients and their families. Studies show it is less expensive than a facility stay, costing up to half as much as a single hospital day. The service is offered to patients with Medicaid or those who want to pay for services out of pocket.

Services may include:

  • Tracheostomy Care
  • G-tube Care and Management
  • Diabetes Management
  • Seizure Management
  • Oxygen Management
  • Coordinating Home Medical Equipment and Supplies
  • Administering Medications and Therapies
  • Personal Care (Bathing, Daily Hygiene, etc.)
  • Educating and Training Family
  • Preparing Meals for Specialized Diets
  • Ventilator Care (Case by Case Dependent)

Emergency Post Hospitilization Medicaid Services

Adaptive 30-day program designed to help prevent unnecessary hospital readmissions.
Adaptive also offers a convenient transition to our long-term homecare program and case management service.
The Adaptive long-term care program allows patients to live independently longer and helps reduce unnecessary hospitalizations and emergency room visits.

Program details:

  • Complete Home Care Coordination and Assistance
  • Coordination with Medicare home health and/or hospice providers
  • Referral Management for other needs such as DME, home modifications, home delivered meals, etc.
  • Continuous Care Plan Oversight provided by a Registered Nurse
  • Daily visits of up to 2 hours by Home Health Aides assisting with:
  • Bathing and bathroom activities
  • Personal Hygiene (hair care, skin care, oral hygiene)
  • Dressing/Grooming
  • Assistance with mobility (walking, light exercise)
Richard

Richard has been with our Bedford office for 2 & ½ years now. Originally Richard and his caregiver, Johnni, were with another company, but he decided to switch providers. Johnni helped him choose Adaptive due to the great conversations with Adaptive staff. Originally just Richard came to Adaptive, but soon after Johnni followed. Richard’s favorite part about Adaptive is Johnni and the awesome office staff.
